Electronic Clinical Quality Measure (eCQM) Standards Landscape: An EHDI Case Study

Clinical quality measures (CQMs) are tools that measure and track the quality of healthcare delivery and services. They can assess the entire healthcare continuum by using data on clinicians’ delivery of care or progress towards long-term quality goals. They can also be used by public health agencies to monitor and improve population health and support decisions related to healthcare quality. eCQMs are electronically specified clinical quality measures. As Early Hearing Detection and Intervention (EHDI) programs advance, having robust measures that enable hospitals and providers to assess the quality and value of the care they provide are essential. Several CQMs, including one eCQM, were developed by the Centers for Disease Control and Prevention (CDC) and endorsed by the National Quality Forum to help track the quality of hearing health care services provided during the EHDI processes. As health information technology standards and CQMs continue to evolve, aligning with updates and changes are an emerging challenge and many interdependencies must be considered when maintaining eCQMs. As a result, new capabilities to manage the complexity of the eCQM environment are necessary. To address these challenges, CDC collaborated with the Integrating Healthcare Enterprise to develop a white paper that summarizes the processes, standards, and technologies involved in the development, use, and maintenance of eCQMs. Using the EHDI-1a (Hearing Screening Prior to Hospital Discharge) eCQM as an example, the white paper explains the complexity of the eCQM standards landscape and presents a vision for decreasing the reporting burden on healthcare facilities and health information system vendors using interoperable data sharing. In this presentation, we will use the EHDI-1a measure as an example to review key components in eCQM development, implementation, and reporting. We will also discuss potential technical solutions to reduce the quality reporting burden and more efficiently manage the complexities associated with eCQMs.

Dr. Xidong Deng is a Health Scientist with the CDC EHDI Team. She holds a PhD. in Computer Science and Engineering from Pennsylvania State University. Dr. Deng’s work with the EHDI team includes helping to develop national EHDI data systems, identifying strategies for integration of EHDI systems with electronic health records and state based systems, evaluation of state based EHDI systems, and the identification of privacy and security solutions for information exchange between state public health registries. Her current interests are in ontology, controlled vocabulary and data standards, EHR and PHR systems, and evaluation of health information systems.
