Decrease Day of Discharge NHS at CHOC Children's NICU

Early hearing detection and intervention in children is not only crucial for language development but it also influences cognition and overall development. In 2002, CHOC Children's Hospital was the first children's hospital in the state of California to implement the state mandated newborn hearing screening program. An infant’s day of discharge home from NICU is the most anxiety laden and stressful event for parents. Performance of newborn hearing screening on the day of discharge further adds to this stress. It becomes even more relevant if the result of screening test done on the day of discharge is 'refer.' Rescreening and or appropriate referral for diagnostic evaluation needs to be arranged on the day of discharge which will not only further delay the discharge process but it will also worsen their stress and anxiety. In addition, it is equally important to perform hearing screening prior to the day of discharge to fulfill administrative expectations. Review of our data reveals that the hearing screening performed on the day discharge was on the rise. In 2016, our infant hearing screening on day of discharge was as high as 25.9%. Therefore, to streamline the hearing screening process and to reduce the day of discharge testing, the following steps were implemented: - Education related to the importance of hearing screening prior to the day of discharge for NICU staff, pediatric residents and neonatal fellows -Implementation of new infant hearing order set. -Adding hearing screening to the NICU discharge checklist -Daily communication between diagnostic services and nursing staff regarding potential discharges -On call technologist availability on weekends and holidays. As a result of the implementation of the listed processes, in 2017 CHOC Hospital NICU decreased the day of discharge hearing screening to 21.4%; which is an overall decrease of 17.4%.
