Prevalence of Hearing Loss among Hospital Births in Michigan, 2009-2013

Nationally, about 1 to 3 infants per 1,000 births are diagnosed with permanent hearing loss. From 2009 to 2013, the prevalence of permanent hearing loss among hospital births in Michigan was 1.8 cases per 1,000 live births . The Michigan EHDI program conducted a study to assess the prevalence of hearing loss among all birthing hospitals in Michigan. The purpose of this study was to compare the prevalence of permanent hearing loss by birth hospital size across the state and to determine whether an association exists between hospital size and the prevalence of hearing loss. EHDI data were obtained from the EHDI Data System for birth years 2009-2013. Live birth data per hospital from the Division of Vital Records and Health Statistics were obtained for the same years in order to calculate hearing loss prevalence. Results were analyzed using SAS 9.2. To determine hospital size categories, we grouped all 84 Michigan birth hospitals into 4 equal categories of size according to the quartile distribution of the average number of births across the 5 years. From 2009 to 2013, the prevalence of permanent hearing loss for small, small-medium, medium and large birth hospitals was 1.2, 1.5, 1.5 and 2.0 per 1,000 live births respectively. Analysis indicated that at least one hospital size differed significantly from another by the prevalence of permanent hearing loss (p=0.0209). Our findings suggest that the prevalence of permanent hearing loss among small hospital births is significantly lower than that of large birth hospitals. Suggested next steps for system enhancement to determine the large-small hospital difference may be a linkage of EHDI data with birth data to examine whether transfer of kids with illness in and out of hospitals may have an impact on the prevalence of hearing loss among birth hospitals.
