QI: Understanding Barriers and Facilitators of Tele-Audiology
North Carolina is one of a small number of states providing screening and diagnostic testing using Tele-Audiology. The State has large, sparsely populated areas that lack proximal pediatric audiologists. Tele-Audiology was initiated as a safety-net to improve the capability of the Early Hearing Detection and Intervention Program (EHDI) to reach babies in rural areas. Underutilization led to the conduct of a Quality Improvement (QI/PDSA) test. The purpose of the PDSA was three-fold: 1) to increase family awareness of Tele-Audiology; 2) to learn from parents of children with hearing loss how best to engage parents in the Tele-Audiology process; and, 3) to discover the cultural nuances that serve as facilitators or barriers to Tele-Audiology services. The study engaged parents and family members from three groups: 1) EHDI Parent Phone Tree (parents that have a child with hearing loss/did not participate in Tele-Audiology); 2) Tele-Audiology participants; and, 3) Spanish- speaking parents that have a child with hearing loss (did not participate in Tele-Audiology). The study used multiple methodologies to gather data: three telephone focus groups; a short telephone survey used with parents unable to participate in a focus group; and a parent satisfaction survey. All measures focused on the same underlying constructs of barriers and facilitators in three domains: family, service provider system, and the community. This process allowed triangulation of the data sets to corroborate the validity of responses. While final data analysis is ongoing, preliminary findings revealed themes that are consistent within groups and distinctly different between groups.
