Minimize Referral Rate for an NHSP

The neonatal intensive care unit at CHOC Children’s Hospital was the first NICU to start state mandated newborn hearing screening program in Southern California in 2002. The referral rate of 10.7 percent for our high risk infant hearing screening program was relatively higher comparing to 8 percent rate for other high risk infant hearing screening programs in Southern California. The referral rate for an NHSP depends on many factors including but not limited to technology used for testing, competency of screeners, behavioral state of infant at the time testing, and ambient noise in the screening area. Therefore we adopted a multifaceted approach to minimize our referral rate. We evaluated our screening processes and quality indicators that lead to identification of improvement opportunities and proposed system changes for immediate implementation. By using multiple PDSA cycles we were able to reduce our referral rate from 10.7 percent to 3.9 percent. This study demonstrates a practical and data-driven approach to quality improvement that can be used by any NHSP.
