Title: 'Loss to Follow up Trends in North Carolina' Track: 6 - Follow-up, Tracking and Data Management Keyword(s): Reducing Loss to Follow Up Learning Objectives: Understand the screening to diagnosis process in NC Identify barriers to meeting 1-3-6 goals for screening/rescreen and diagnosis 3. Acknowledge the importance of Acknowledge the importance of stakeholders involvement in the rescreen process. Abstract: EHDI staff worked with a region in North Carolina that traditionally had a high loss to follow up rate and difficulty meeting 1-3-6 goals. From 2009-2010 we made a concentrated effort to target this region with a NICHQ (National Institute for Children’s Health Quality) project to reduce loss to follow up. The project targeted one birthing facility that did not provide outpatient rescreens to babies that did not pass the initial screen. Following completion of the NICHQ initiative we worked with that specific hospital to establish rescreen procedures. As a result of these efforts, lessons learned were extended to other birthing facilities within the same hospital network in the same region. We have seen a reduction in loss to follow up for this county from 2009 to 2012.
