Abstract:

Research reveals continued barriers to identification/treatment of hearing loss among low socio-economic status (SES) and ethnic minority populations. A study of Massachusetts’ “lost to follow-up” populations found that people with public insurance had lower follow-up rates than those with private insurance. In the same study, it was found that infants born to racial or ethnic minorities are at a higher risk of becoming lost to follow-up (Liu, Farrell, MacNeil, Stone & Barfield, 2008). Another study found that treatment of children with otitis media (OM) varies based on socioeconomic level and that the most common identified risk factor for OM is socioeconomic status (Smith & Boss, 2010). Significant challenges remain beyond Universal Newborn Hearing Screening in identifying and treating hearing loss among at risk populations, as well as an overwhelming need for additional programs and resources. To address these needs, a hearing screening program was created in an urban area. The goal of the program is identification of hearing loss and resource dissemination. To effectively meet the challenges facing this population, the program has developed a unique culture of cooperation between the medical clinic, government agencies and community systems. Through its use of interpreters, cultural brokers and unique strategies for providing patient and family-centered services, the program is a model for interdisciplinary and culturally-sensitive collaboration. Additional information will be shared about how to incorporate URLEND trainees in this type of program. Through learning about this unique hearing screening program, learner outcomes include understanding the Medical Home model in action and how to implement this strategy in other communities and clinics.
