Abstract:

The goal of the Louisiana Early Hearing Detection and Intervention Program (LA EHDI) is to ensure that all newborns are screened for hearing loss so that those with a diagnosis of permanent hearing loss can achieve their maximum potential. LA EHDI promotes the national EHDI goal of screening for hearing loss by 1 month of age, preferably before hospital discharge. To define newborns who received late initial hearing screening tests performed in the hospital, LA EHDI initial hearing screening data that were linked with Louisiana birth certificates of newborns born in 2011 were analyzed. The timeframe when a newborn received hearing screening prior to hospital discharge was calculated and compared between subgroups in each maternal and child’s characteristic using linear regression models. The timeframe when a newborn received hearing screening was defined as the number of days between the date of birth and the date of screening before hospital discharge. The study showed that newborns who had very low birth weight, were very preterm, admitted to NICU, or had a low Apgar score received late screening in the hospital prior to discharge. For example, the timeframe was greater than 30 days among newborns with very low birth weight (VLBW, < 1,500 grams) or those born very preterm (VPT, < 32 weeks of gestational age). The timeframe was about 22 days longer among newborns with VLBW than among normal birth weight newborns; and 17 days longer among those born VPT than among term birth newborns. These characteristics have been also defined as risk factors of hearing loss from previous studies. Therefore, effective plans and strategies to improve adverse birth outcomes and delivery status of newborns will identically improve late receipt of initial screening as well as reduce risk factors of hearing loss.
